I hereby allow my child(ren) to participate in Christian Home Educators Academic Resouce or CHEAR and hereby declare as follows:

I understand that certain activities engaged in while my child(ren) participates/participate in activities, studying, tutoring, or learning may involve inherently dangerous or risky activities. While CHEAR (including its officers, directors, employees, contractors, or volunteers) will make reasonable attempts to foresee and avoid any such risks (or warn me of such), these risks may be either unavoidable, unknown, or merely occur in the course of activities related to my child(ren)’s participation in CHEAR activities. I understand, acknowledge, and agree that it is solely my responsibility as my child(ren)’s parent to supervise my child(ren). It is not CHEAR’s responsibility and, accordingly, by signing this document I agree to be completely and solely responsible for any injury or damage my child(ren) incurs/incur to any extent. As the parent or legal guardian of my child(ren), I assume all the risks associated with my child(ren)’s participation and waive, release, and forever disclaim CHEAR, its board, officers, employees, volunteers, and independent contractors from any liability related to my child(ren)’s participation in CHEAR events.
CHEAR has my permission to provide or to obtain medical treatment for my child(ren) when I cannot be reached, or if a delay in reaching my child(ren) would result in serious injury or bodily harm to him or her. I understand that medical decisions may or may not be made by a medical professional.
I understand that 9-1-1 will be called at the discretion of the CHEAR Activity Coordinator/Sponsor, and the parent will be responsible and assume all expenses. I understand that while I may have a preferred medical treatment center, this decision will be left to the discretion of responding EMS.
I understand the only medications that will be administered while my child(ren) is/are attending CHEAR activities will be Benadryl for life-threatening allergic reactions or other medications that I provide for my child(ren).
I understand that medical responsibility falls primarily on the parent/legal guardian and not on the CHEAR Activity Coordinator/Sponsor. It is my responsibility to assure my student will be picked up by me or a designated adult within one (1) hour after being notified by the CHEAR Activity Coordinator/Sponsor team.

By signing below, I understand that I assume all financial responsibility for any treatment or injuries sustained by my child(ren) while he or she is under the supervision of CHEAR. 10. CHEAR Liability Release

I, the below signed adult, do hereby release from liability and hold harmless Christrian Home Educators Academic Resource (also known as CHEAR) and any of its employees, members, or agents as regards the usage of the facilities and in the event of any accidents in route to, during, or returning from any CHEAR-sponsored events.

This release is for any and all liability for personal injuries (including death) and property losses or damage in connection with any CHEAR activity, including Scholastic Co-op, physical activities, etc…. The undersigned further agrees to abide by all the rules and policies of CHEAR.

I hereby consent to receive medical treatment for myself or member of my family which may be deemed advisable in the event of injury, accident, and/ or illness during a CHEAR event. This treatment could include treatment by a licensed physician, x-rays, any prescription drugs, or surgery.

I understand and agree that the primary responsibility for medical coverage rests with my personal medical insurance and with me. In the event medical treatment is necessary, all hospital and physician's bills shall be directly billed to me. I assume liability for any resulting expense, which is not covered by my personal insurance.

I certify that I have read this document and I fully understand its content. I am aware that this is a release of liability and a contract, and I sign it of my own free will. I further state that I understand this consent to extend to all members of my family who might attend CHEAR functions.

In the 2025-26 school year, each family participating in the CHEAR Scholastic Co-op will be required to complete 24 service hours total during the school year. Parents are required to attend the service hours that they have signed up for.

What do I do with littles or kids not enrolled?

We will pick a position suitable to accomodate the kids that are with you on shift.

Can I pay someone to cover my hours rather than swap?

Yes, parents must reach out to another member of CHEAR to swap.

Who can cover my shift?

Only adults listed in your profile OR a member of the co-op can cover your hours.

We understand that emergencies happen. We highly advise that you have 2-3 backups in place always to cover your hours when there are emergencies so you don’t end up in a bind and get charged a fee. Typically in our group, parents swap service hour dates if they are unable to serve. They will work out an agreement on a date to swap and report it to the Service Hour Coordinator. The best way to find a replacement is by calling, texting, and utilizing our private member Facebook Group to find a swap. However, ifyou do not make finalarrangements forasubstitute to fill your commitmentat least 24 hours prior to your scheduled date, a fee will be applied to compensatethe parent who steps in to cover theshift.

Service Hour Compensation Rates: 4-Hour Shift: $60 3-Hour Shift: $45 2-Hour Shift: $30 Each missed shift will incur a $15 increase per week, up to a maximum of three weeks, if coverage is not arranged. If fines accumulate up to three times, your membership for the following year may be at risk. Please make every effort to fulfill your shift responsibilities to avoid accumulating fees and potential un-enrollment. Every parent plays a vital role in keeping our group safe and secure! Our Service Hour coordinator is not responsible for finding your substitute for your service hours

14. Payment and Fees 25-26

I/We pledge our support to the CHEAR Scholastic Co-op through PAYMENT OF FEES and ON TIME TUITION PAYMENTS. I/We will assist our child(ren) in being prepared for class and in any assignments. If questions arise, I/we will contact the tutor FIRST, then if necessary the Administrator. I/We understand that the Co-op is a parent cooperative and will work to make the cooperative work for our own family as well as for the families of all other students. Late fees will incur a fee of $10 per week.

Tuition is due at the first Wednesday of every month for a total of 8 equal payments. You are welcome to pay for the year or semester.

15. Badges 25-26

CHEAR and Church 360 require badges for security purposes. Wearing your badge at all times while on campus is essential to maintaining the safety standards established through our collaborative efforts.
Failure to wear your badge compromises these efforts. If you forget or lose your badge, a temporary badge will be issued for the day. After two instances of receiving a temporary badge, a $5 replacement fee will be invoiced, and payment will be required immediately for a new permanent badge.

16. Add/Drop Policy

Dropping a class from co-op is strongly discouraged, as the expectation between tutors and parents is a year-long commitment. However, we understand that circumstances may arise. It is the parent’s responsibility to communicate directly with current or future tutors to finalize any class changes. The following steps must be followed for any add/drop requests:

Submit the Online Request: Go online and complete the Request to Drop form so the involved tutor(s) receive a notification that you have a change. Then coordinate a meeting with the tutor(s) to discuss the change.
Meet with the Tutor(s): Pick up an official Add/Drop Form from the admin table on Wednesday. Meet with the tutor(s) on campus during Wednesday co-op hours to obtain their signature(s) approving the change.
Submit the Form: Turn in the completed and signed Add/Drop Form to the admin at the co-op.
Await Confirmation: The Registrar will email you with a confirmation and an updated invoice for the fees associated with the change.
Please note: Your student is not officially enrolled in or dropped from a class until you receive the Registrar’s confirmation. Do not attend the new class until approval is finalized.

17. Illness Policy 25-26

I/We agree no member of our family will attend CHEAR events while knowingly ill. Every child and adult in our household must be free of all symptoms for at least 48 hours before participating in any events. Symptoms include: Fever (temperature over 100.0 degrees F), diarrhea, vomiting 2 or more times in the past 24 hours, body rash with fever, severe coughing, eye discharge, or any other communicable illness.

If any person arrives with a temperature over 100.0 degrees F, they will not be allowed in the building. If a student developes any of the above symptoms while at CHEAR, the child will be sent to a designated sick room and the parent will be notified immediately. 18. Statement of Faith

This co-op welcomes participants ofany denomination to engage in classes, as we believe that weareall one in Christ Jesus (Galatians 3:28). Tutors must teach in a manner that honors Biblical principles, recognizing that the fear of the Lord is the beginning of wisdom (Proverbs 9:10),and that we should do everything in love (1 Corinthians 16:14). We believe that the Bible is the finaland onlysource ofauthorityin our lives, as it is God-breathed and useful for teaching, rebuking, correcting and training in righteousness (2 Timothy 3:16). Our corevaluesand character traitsare rooted in the Bible, and we believe that our faith in Christ is essential to our successas homeschoolersand as individuals, as we trust in the Lord with all our heartsand lean not on our own understanding (Proverbs 3:5-6). Weare committed to deepening our understanding of the Bibleand applying its teachings in all areas of our lives,as the Word of God is living and active, sharper than anytwo-edged sword (Hebrews 4:12). 19. Sanctity of Life, Gender, Sexuality and Biblical Marriage

CHEAR is a Christian co-op guided by biblical principlesand values. Our beliefs regarding marriage, sexuality, gender, and the sanctity of lifeare firmlyrooted in Scripture. We believe that God established marriageasacovenant between one man and one woman (Genesis 2:24, Matthew 19:4-6)and that sexual intimacyisa gift from God, intended to be enjoyed within the context of marriage (Hebrews 13:4, Song of Solomon 4:12-5:1). Weaffirm that God created each person with a unique gender identity, intrinsicallylinked to their biological sex(Genesis 1:27, Psalm 139:13-14). Additionally, we believe thatall human life is sacred and should be protected from conception until natural death (Psalm 139:13-16, Jeremiah 1:5). These biblical convictions shape ourapproach to education, influence our policiesand practices, and guide our interactions within the CHEAR community. 20. Cancellations

As a cooperative, we work together to create a meaningful and enriching experience for our children. For our program to run efficiently and safely, it is essential that all parent volunteers and tutors arrive on time and actively participate in their roles. In the event of severe weather, insufficient volunteer coverage, or circumstances beyond our control, we may be required to cancel co-op for the day. These decisions are made by the co-op leadership and may differ from local ISD closures, as our families reside across a wide geographical area. Please note that tutors structure their tuition rates to account for occasional cancellations and are not required to issue refunds for missed days. Additionally, our co-op schedule does not include makeup days for canceled sessions.
